Request for Sacramental Information

Sacramental records are important to an individual's identity, both in the Church and in society. The information contained in the records is the property of the individual who received the sacrament and the Parish. Our Parish has adopted the following guidelines for the release of sacramental information.

As there are privacy issues determining the right of access to Church records, requests for verification of sacraments must be received in writing using the "Request for Sacramental Information from Church Register" form which can be downloaded HERE. This form must be completed in its entirety and submitted via email or in person to the Parish Office.

Sacramental information verifying the reception of Baptism, Confirmation, First Communion or Marriage can only be released to the following persons:

  1. The individual who received the sacrament (aged 18 years or older);
  2. A parent named in the record of a child under the age of 18 years; or
  3. The legal guardian of a child under the age of 18 years (proof of guardianship is required).

The information will be prepared and emailed within 14 days to any of the above or can be collected from the Parish Office by any of the above or by a person delegated by the requestor (proof of delegation is required).
